Post Acute Rehabilitation at Bella Vista Health Center

Post acute rehabilitation services are designed for patients of all ages who no longer need hospital care, but would benefit from additional therapy and care before returning home following injury, illness, or surgery. At Bella Vista Health Center, we provide a continuum of care that bridges the gap between hospital and home for those in need of short-term rehabilitation or long-term care.

Our San Diego post acute rehab teams are committed to helping patients achieve their recovery goals and regain their quality of life.  We partner with patients and families to create customized post acute rehabilitation programs that incorporate individual preferences and goals in order to help patients meet their highest level of functioning.

Post Acute Rehab Center Programs

At Bella Vista Health Center we understand that transitions can be challenging. In order to address both the physical and emotional needs of each patient, our post acute rehab teams include skilled therapists, psychologists, physicians, nurses, caregivers, dietitians, and social workers.

In addition to administering medications, performing wound care, and other medical services, our specialized post acute rehab team members provide physical therapy, occupational therapy, and/or speech therapy to maximize function and help patients:

The goals of post acute rehabilitation vary by patient. Those suffering from burns, fractures, and brain injuries aim for full recovery, while others need help managing chronic conditions or adapting to daily living after a stroke or the loss of a limb. We provide a range of rehab programs that incorporate exercise, strength training, pain management techniques, adaptive equipment, and more along with specialized medical care to address each patient’s needs.

Post acute rehab goals might be unique to each patient, but our goal is the same for everyone: we want to help every patient live as independently as possible and maximize their quality of life. For more information about our post acute rehab services, click here.
